Adelaide PHN Health Equity Award
Supported through Adelaide PHN’s Flourish Primary Care Innovation Grant, Pasadena Medical was recognised with a Health Equity Award for our work to improve chronic condition management and health equity through community engagement.
The initiative was led by Practice Manager Rachel Morgante and Nurse Manager Meg Hall, with a focus on improving access to chronic disease management for culturally diverse communities, Aboriginal and Torres Strait Islander peoples, and people experiencing socioeconomic disadvantage.
Through tailored care planning and meaningful community engagement, the project aimed to empower patients to better understand and manage their health, while also increasing engagement with and uptake of essential healthcare services.
The project reflects our belief that achieving better health outcomes is about more than simply providing access to care. It requires understanding the unique circumstances, needs and barriers experienced by different members of our community, and finding practical ways to make healthcare more accessible and meaningful.
We are incredibly proud that this work was recognised through the Health Equity Award and grateful to Adelaide PHN for supporting the initiative through the Flourish Primary Care Innovation Grant.
Most importantly, we are proud of the impact the project has had for our patients and community, and of the work of our team in continuing to find innovative ways to improve access, equity and health outcomes.
General Practice Manager of the Year & National Practice Manager of the Year 2025
Pasadena Medical is proud to celebrate the achievements of our Practice Manager, Rachel Morgante, who was awarded both General Practice Manager of the Year and National Practice Manager of the Year through the Australian Association of Practice Management (AAPM).
These awards recognise excellence in practice management and acknowledge the leadership, innovation and contribution of practice managers to the delivery of high-quality general practice.
For Rachel, the awards represent a commitment to continually improving the way Pasadena Medical operates — strengthening systems, supporting our clinical and administrative teams, driving quality improvement and creating a positive experience for our patients.
While these awards recognise Rachel’s individual contribution, they are also a reflection of the team behind Pasadena Medical. Effective practice management is a collective effort, and these achievements would not be possible without the dedication, expertise and commitment of our entire team.
We are incredibly proud of this recognition and of the continued focus on excellence, innovation and continuous improvement that it represents.
